LICENSE RENEWAL

On-line renewals are no longer available as of 12:00am November 1, 2007.  You must renew using your Renewal Form that was mailed to you in September.  If you do not have the form you can e-mail our office requesting that we either mail or fax a form to you.  Please allow 2 business days to generate and send the form.

Reminder Notice: Your Nevada Pharmacists license
expired in October 

Your license with the Nevada State Board of Pharmacy expired October 31, 2007.  Renewal applications were mailed the week of September 17th, 2007, if you did not receive it by October 1st please call us at (800) 364-2081. 

Please note, renewal applications that are sent in without being 100% complete will be returned and not considered to be received until they arrive at the board office with complete information.  If postmarked after October 31, 2007, a $125 late fee will be assessed; otherwise the renewal fee is $150. 

Please check the address we have on file for you by verifying your license on-line with your license number only or your last name only.  If your address has changed, please provide the new information to us on-line at bop.nv.gov (no www), by fax at (775) 850-1444, or mail to Nevada State Board of Pharmacy, 555 Double Eagle Ct #1100, Reno, NV  89521. 

Pharmacists living and practicing  in Nevada must obtain one CEU in a jurisprudence program approved by the Board.  DO NOT send any continuing education certificates unless you received an audit letter.  CE certificates must be dated between November 1, 2005 and October 31, 2007.

 

NOTE: Any changes that you have made to either your name and/or address will be updated prior to your certificate being mailed out. Please allow 10 business days for your certificate to be mailed out.
