The Board of Pharmacy website is not accepting on-line renewal of licenses at this time. If you would like to renew your license please contact the board office either by e-mail (pharmacy@pharmacy.nv.gov) or by fax (775) 850-1444 or by phone (775) 850-1440. We can either mail or fax the form to you and you can mail the complete form and a check to the board office.

Please note, renewal applications that are sent in without being 100% complete will be returned and not considered to be received until they arrive at the board office with complete information.  If postmarked after October 31, 2008, a late fee will be assessed.

Please check the address we have on file for you by verifying your license on-line. If your address has changed, please provide the new information to us on-line at bop.nv.gov by clicking on the tab titled Change Licensee Information. Please allow 10 business days for the change to appear.
