Renewal forms
will be mailed on September 15, 2009.
You will need the form to renew on-line
as it contains your log-in and password
information. The board office does not
have this information readily available
so you must wait for the form to
arrive. If you have not received
your form by October 1, 2009 then please
call and we will send you another one.

Please note,
renewal applications that are sent in
without being 100% complete will be
returned and not considered to be
received until they arrive at the board
office with complete information.
If postmarked after October 31, 2009, a late fee will be assessed.
If your address
has changed, please provide the new
information to us on-line at
bop.nv.gov
by clicking on the tab titled Change
Licensee Information. Please allow 10
business days for the change to appear.
